New Patient Appointments
Please note that you must confirm that you will be showing up to your new patient appointment 2 days prior to your appointment date. If you do not confirm 2 days prior, your appointment will be canceled. Due to the high volume of patients needing appointments, it’s important to confirm your appointment with our clinic.
New Patient No Show Policy
If you are a new patient and do not show for your first appointment without calling to cancel, you will not be rescheduled. You will be dismissed from our practice.
Cancellation Policy
We understand that occasionally you may need to cancel your appointment. Please provide us with a 24 hour advanced noticed so that your appointment may be given to another patient. Failure to notify the office 24 hours in advance will lead to being marked as a “No Show.”
No Show Policy
Patients who schedule appointments but fail to show up are documented as “no shows.” Patients who fail to cancel an appointment 24 hours prior to their appointment will be documented as a “no show.” After your second No Show/No Call you will be dismissed from the practice.
Late Arrival Policy
If you arrive 15 minutes or later for your scheduled appointment, you may be asked to reschedule. Prompt arrival for your appointment allows us to see you and other patients in a timely manner and keeps our healthcare providers on schedule.
Medication refills/Paperwork requests
There is a 24-72 hour waiting period on ALL medication refills. It is best to notify your nurse at your appointment if you need any refills.
Providers require a minimum of 14 business days to complete any paperwork. Physical, school and work forms may require an office visit prior to be completed.
Copayments and Payment Policies
Copayments and balances are due at the time of your appointment and will be collected at check in. Accepted methods of payments are cash, checks, and all major credit cards except American Express. You can contact our Billing department at 315-464-2000 with any additional questions
